Chest surgery (Breast Surgery)

Chest surgery (Breast Surgery) is a surgical procedure on the chest to alter the size or shape of the breasts. In some cases, it is done to address health issues, while in other cases, it is performed for aesthetic purposes—such as desiring a different size or a more pleasing shape. There are several surgical techniques depending on the purpose of the surgery and other factors, such as the patient’s goals. Examples include breast reduction or removal for gender transition from female to male, which is a common procedure among women and transmasculine individuals.

FTM Top Surgery (Top Surgery for Female-to-Male) — What is it?

FTM Top Surgery is a procedure to reshape the chest so that a trans man’s physique appears male. The surgeon removes breast tissue, excess skin near the armpits, and performs liposuction to refine the chest contour to match the patient’s body proportions. The nipples and areolas are also reshaped and repositioned to resemble those of a typical male chest. This is one of the most popular gender-affirming surgeries for female-to-male transitions.

Who is Chest Surgery Suitable For?

Top surgery is suitable for individuals who wish to remove breast tissue to achieve a flat, masculine chest appearance. Patients are usually trans men or female-to-male transgender individuals, but the procedure can also help men with gynecomastia (enlarged male breasts).

Pre-Surgery Requirements for Chest Surgery

To undergo top surgery in Thailand, patients must meet the Medical Council of Thailand’s requirements. This includes a physical examination and obtaining letters from two psychiatrists confirming readiness and understanding of the physical changes after surgery.

Pre-Surgery Instructions

  1. Consultation with two psychiatrists

  2. Two blood tests

  3. RT-PCR test 2 days before surgery

  4. Chest X-ray (can be done at a local hospital and submitted to the clinic)

  5. Stop hormones, smoking, alcohol, supplements, and all vitamins 2 weeks before surgery

  6. No food or drink for 8 hours before surgery

Post-Surgery Care for Chest Surgery

  1. Keep surgical drains for about 3–7 days, or until bleeding has stopped for more than 2 consecutive days

  2. Sutures removed 14 days post-op

  3. Keep the wound dry for 14 days (no showers, sponge baths only while wearing the chest binder)

  4. Do not remove the chest binder until instructed by the doctor

  5. Continue wearing the binder for 3 months to help shape the chest

  6. Avoid heavy exercise, lifting, or raising arms overhead for 6 weeks post-op
