Facial Liposuction

Facial liposuction has become increasingly popular nowadays because it differs from fat-dissolving injections (such as fat-melting solutions). Facial liposuction provides more satisfying results since it removes fat directly and precisely. However, it is necessary to wear a chin compression garment after the procedure to allow the skin to adhere smoothly to the underlying tissue, as the skin in the treated area will have been separated using surgical instruments.

Choosing the right method for facial fat reduction
Because the face is an area with a higher risk of nerve injury during liposuction, the fat reduction method should be selected based on the specific location to achieve the best results while avoiding side effects.

  1. Buccal fat – This is fat located beneath the muscle layer, adjacent to the inner cheek inside the mouth. Surgical removal of buccal fat is recommended for this area, as it will cause the outer cheek to reduce in size. (If liposuction is used instead, the skin may sag or become uneven.)

 2. ​Jawline fat – This fat is located above the jawbone and can be effectively treated with liposuction, resulting in a more defined jawline and being considered a safe approach.

Chin and jawline liposuction – Permanent fat cell removal – Who is it for?
Before performing chin liposuction, doctors at the clinic will first assess whether you are truly suited for this treatment or whether another method should be combined for optimal results.
In most cases, liposuction alone is sufficient if you meet the following criteria

  1. Excess fat under the chin and jawline – Fat that exceeds normal limits, appearing as a bulging pouch under the chin, making the jawline unclear. This fat often extends toward the neck. The doctor will measure the fat precisely before treatment.

  2. Firm skin – Skin that is not overly loose or saggy. Younger patients usually do not have issues here, but older patients with sagging skin may need a neck lift or skin tightening treatments such as Thermage or Ulthera in combination with liposuction.

  3. Neck muscles are not overly thick – The platysma muscle in some older patients may appear as thick vertical bands along with sagging skin. In such cases, muscle tightening surgery may be required. However, if the neck skin is smooth and there are only minor lines, this is normal.

  4. Proportional chin and jawbone structure – Patients with a well-proportioned chin and clearly defined jawbone will get the best results from liposuction. If the chin is receded or short, a chin implant may be recommended.

Preparation for local anesthesia cases

  1. Get enough rest the night before.

  2. Eat a meal 3 hours before coming to the clinic.

  3. Avoid tea, coffee, and carbonated drinks to reduce frequent urination.

  4. Wear loose, dark-colored clothing (there may be some fluid leakage from the incision site afterward).

  5. Bring a friend or family member. Do not drive yourself home.

Preparation for general anesthesia cases

  • Get at least 8 hours of rest the night before.

  • No food or drink for at least 8 hours before the procedure.

  • No smoking for at least 8 hours prior.

  • Avoid tea, coffee, and carbonated drinks.

  • Do not wear makeup or apply skincare products.

  • Remove all jewelry.

  • Cut nails short and remove polish from at least one nail.

  • Wear loose, dark-colored clothing.

  • Bring a friend or family member; do not drive yourself home.

Ideal chin, neck, and jawline proportions
A youthful lower face and neck are assessed using several criteria known as the “Youthful Neck” profile

  1. Cervicomental angle (angle between chin and neck) between 105–120 degrees.

  2. A well-defined jawline (distinct inferior mandibular border).

  3. Smooth, firm skin without sagging.

  4. No visible neck lines.

  5. No visible platysma muscle bands.

  6. No enlarged submandibular glands visible under the jawline.

  7. Non-enlarged masseter muscles.

360 Liposculpture technique for chin and jawline liposuction

  1. Detailed planning – The doctor assesses chin fat considering the entire face and neck, ensuring the fat volume is appropriate for safe and effective removal. If liposuction is not suitable, other personalized treatments will be recommended.

  2. Tumescent local anesthesia – minimal pain – Anesthetic mixed with sodium bicarbonate (NaHCO₃) and adrenaline is used, reducing stinging and controlling bleeding while prolonging anesthesia. Pain level is usually only 1–2 out of 10, so general anesthesia is unnecessary for this area.

  3. Ultrasound-assisted microcannula liposuction – minimal swelling and bruising, no nerve damage – Only an adequate amount of fat is removed to avoid irregularities or overly thin skin. Intermediate-to-deep plane liposuction is used to keep a thin layer of fat under the skin for smoothness. Ultra-Z technology is used to precisely break down fat without overheating or damaging nerves, while also stimulating collagen production for skin tightening.

  4. Special jawline refinement technique – safe and precise – This area is challenging due to natural ligament attachments (mandibular ligament) and proximity to nerves. The tumescent hydrodissection method is used to safely access and remove fat through a small incision under the chin.

  5. Tiny incision with minimal scarring – A 3–4 mm incision is hidden under the chin, allowing full access to the target area. The incision is protected with a port to prevent friction and is closed for optimal healing with minimal scar risk.

  6. Smooth and refined results – The process focuses on overall contour rather than the maximum amount of fat removed, ensuring a balanced and aesthetically pleasing outcome.

Important notes before chin liposuction

  1. Factors such as skin condition, facial and neck muscles, bone structure, and fat thickness should be assessed first. Consultation is recommended.

  2. Healthy individuals can undergo the procedure safely, but should stop taking blood-thinning vitamins a week before.

  3. Wearing a compression garment as much as possible—especially in the first 3 days (12–24 hours/day)—is essential for optimal skin retraction. After 3 days, wear it 8–12 hours/day for 1 month.

  4. Common after-effects include mild swelling for 3 days, slight bruising (yellow-green or purple) that resolves in 1–2 weeks.

  5. Most patients can return to work after 2–3 days, depending on swelling.

  6. Firmness under the chin 2–3 weeks post-op is normal and improves after 1 month, especially with RF massage.

  7. At our clinic, light therapy and skin tightening massages help speed recovery and reduce fibrosis.

  8. Additional Thermage treatment 2 months after surgery can further tighten skin, stimulate collagen, and reduce fine lines.
